2025–2026 年的重要改版
這一年半變動最多的幾件事
- ACS：2025 ACC/AHA 新版取代 2013 STEMI／2014 NSTEMI／2015 primary PCI 更新——橈動脈優先、左主幹與複雜病灶用血管內影像、STEMI 完全血管重建、高強度 statin 可同時起始 ezetimibe、ticagrelor 單方降階。
- 血脂：2026 ACC/AHA 血脂指引取代 2018 膽固醇指引——次級預防高風險 LDL-C <55 mg/dL、non-HDL-C <85 mg/dL；Lp(a) 一生至少測一次。
- 心臟衰竭：2026 ESC 指引移除 HFmrEF（HFrEF 定義為 LVEF <50%）、四分期、「acute HF」改稱「decompensated HF」；2026 第二版全球共通定義發布；美國新版指引預計 2027 年底。
- 中風：2026 AHA/ASA 急性缺血性中風指引取代 2018／2019 版——tenecteplase 與 alteplase 並列、灌流影像下的延長時間窗、大核心與基底動脈血栓移除、不建議積極降壓至 <140 mmHg。
- 瓣膜：2025 ESC/EACTS——更早介入、TAVI 年齡分界由 75 下修為 70 歲。
- 心肌炎與心包炎：2025 ESC 首份合併指引。
- 心因性休克：2025 ACC 首份 Concise Clinical Guidance，採用 SCAI 分期與 SUSPECT CS。
- 高血壓：2025 ACC/AHA 新版（2017 年後首次大改版），目標 <130/80、採用 PREVENT 風險方程式。
- 左心耳封堵：2025 SCAI/HRS 首份多學會指引；2026 focused update 進行中（截至 2026-09 仍在公眾意見階段）。
